VIAL OF LIFE PROGRAM: The Vial-of-Life program was started for the residents of Sycamore Township to help enhance emergency medical treatment, especially if you are unconscious or unable to communicate. The FREE vial-of-life is a plastic container that holds vital medical information about the occupants of each residence. One information sheet per each residence is to be filled out and placed in the vial (extra information sheets are available at the fire department). The vial is then kept on the top shelf of the refrigerator and a vial-of-life magnet goes on the door of refrigerator to alert the paramedics of the information. Please keep all your vital information current on the information sheets. In an emergency situation it is hard to think of everything that the emergency personnel need to know. All your vital information is recorded on the information sheet including your medical history, doctor's names and phone numbers, emergency contact person, prescription drug information, medical coverage, and hospital preference. With all the information recorded on the vial-of-life information sheet, this saves valuable time in caring for the patient. These facts are very important during a life-threatening situation. If the patient is transported to the hospital, the emergency medical personnel will take this information with them to the hospital so as not to delay treatment. |

The Sycamore Township Fire Department sells the green reflective signs for $9.00 (sign only) and $14.00 (sign & post) each. The numbers on the signs can be placed horizontally or vertically.
